Please be sure to bring your insurance information, a referral slip from your doctor, and a completed Medical Information Form which you should have received from our office. It is most helpful to the doctor that you bring all the medications you take (prescription and non-prescription) – a list is helpful, but the medications in their bottles is even better – and any medical records which you may have.
